Job Title: Director of Rehabilitation (DOR)

Employment Type: Permanent, Full-Time

Location: Hayward, CA

Shift: Day Shift | Monday-Friday, 8:00 AM - 4:30 PM

Position Summary

We are seeking a Director of Rehabilitation (DOR) to oversee and manage the daily operations of the rehabilitation department in a skilled nursing facility. This leadership role is responsible for supervising a multidisciplinary therapy team and ensuring the delivery of high-quality, compliant, and patient-centered therapy services. The DOR will be an integral member of the facility's leadership team, driving strong clinical outcomes, regulatory compliance, and staff development.

Key Responsibilities

• Lead and supervise physical, occupational, and speech therapy teams.
• Ensure therapy services meet all regulatory requirements and facility policies.
• Manage departmental scheduling, productivity, staffing, and workflow.
• Oversee documentation accuracy, billing processes, and reimbursement practices.
• Collaborate with interdisciplinary teams to develop individualized treatment plans.
• Promote a positive and collaborative team culture through coaching and mentorship.
• Maintain compliance with Medicare, Medicaid, private insurance, and PDPM guidelines.
• Participate in quality improvement initiatives and audits.

Qualifications

• Licensed Physical Therapist (PT), Occupational Therapist (OT), or Speech-Language Pathologist (SLP) in California (required).
• Minimum 3 years of clinical experience in a skilled nursing or rehabilitation setting.
• Prior leadership experience as a DOR or Rehab Manager strongly preferred.
• In-depth knowledge of Medicare regulations, PDPM, and long-term care standards.
• Excellent organizational, communication, and leadership skills.
